Each service is tailored to fit the unique needs for all individuals and their loved ones.

Who Would Benefit from The Mourning Moonlight?

Those who use services earlier in the process of end-of-life benefit longer from our services. Early planning before health decline helps establish a plan that can be set into action if there is an unexpected decline in one's health. Here is a list of individuals The Mourning Moonlight can work with. Some individuals may fall under multiple categories.

Chronic Illnesses

Families with a loved one who has a chronic or terminal Illness

Terminal Illnesses

Planners

Some Examples:

COPD, Congestive heart failure, kidney disease, autoimmune diseases, kidney diseases, and some cancers

If You have a chronic illness and you have had the following:

  • Difficulty managing

  • Progression of illness or health decline

  • No longer wishing to manage illness or make it a priority

  • Needing assistance managing at home

  • Referred to hospice

Some Examples:

Advance cancer, dementia (including Alzheimer's), Neurological diseases, and advance lung and heart diseases

  • Just been given diagnosis

  • Progression of illness despite treatment

  • Referred to hospice

  • Needing assistance managing at home

  • Rapid decline in health

If You have a terminal illness and you have had the following:

  • If you are a caregiver for someone who has any of the following listed above and is needing assistance with end-of-life planning or care.

  • If your loved one is actively passing and you are needing support and assistance

These are individuals who are in fair or good health but would like the following:

  • Establish plans for end-of-life care

  • Discuss medical wishes with family

  • Assistance with getting assets in order

  • Assistance with coming to terms with mortality

  • Assistance with improving quality of life

This Includes:

  • 6 visits (zoom or in person)

  • Assistance with getting assets in order (paperwork, funeral planning, working on forgiveness and making peace)

* NOT ABLE TO GIVE LEGAL ADVICE

  • Discussing wishes for medical care

  • Education on progression of illness and how to manage

  • Organizing a plan of care

  • Education on caring for loved one with illness

  • Hospice planning

  • Education on End-of-Life

  • Collaboration with hospice or Primary Physician

  • Providing a space to help navigate difficult conversations between you and loved ones

  • Living funeral

  • Legacy project

This Includes: 

  • 16 visits (zoom or in person)

    *These days can be condensed for longer days/support

  • Hands on nursing care

  • Education on End-of-Life and how to care for those in this stage

  • Holding space to help navigate difficult discussions with you or loved ones

  • Vigil

  • Caregiver relief

  • Collaboration with hospice or Primary Physician

  • Post death care

  • Legacy Project

  • Living funeral

End-of -Life Support (6 months)
