Three-Month Planning Package


For individuals or families ready to begin planning with clarity and structure.

  • 6 sessions across 3 months (2 per month)

  • Guidance with advance care documents & estate planning

  • Roadmap for possible future care needs

  • Referrals to legal, medical, and financial resources

  • Emotional support to ease difficult conversations

Six-Month Comprehensive Package


For those seeking more in-depth support over an extended time. Ideal for families with complex needs or individuals wanting additional guidance.

  • 12 sessions across 6 months (2 per month)

  • Deeper exploration of estate planning and care options

  • Ongoing support through health changes or family transitions

  • Continued guidance to adjust plans as needed

  • Emotional and caregiver support throughout the process

Single Services

  • Registered Nurse Visit
    Per-visit nursing support including assessments, medication management, and hands-on care.

  • Consultation Session
    60–90 minutes, in-person or virtual, for tailored planning or caregiver guidance.

  • Vigil Support (End-of-Life Presence)
    Compassionate bedside support and nursing care during the final hours or days of life, ensuring comfort and peace for both the individual and their loved ones.
